Okay, I know what you're thinking... I'm not in medical school this won't be interesting, but it WILLLL! These cases that I'll highlight quickly and show some pictures were mad cool. Read ON! For those of you who are medical... cut me a little slack, okay! Remember, this is me translating/interpreting from the italians... i think I did okay, but the lingo isn't perfect! here's five crazy cases for you... * 56 y/o M. 4th time re-op on a infiltrating Medullablastoma in posterior cranial fossa, L cerebellum. hemispheric approah. s/p chemo and XRT regimens. The chairman did this surgery and he was skin to skin in one hour... it was phenomenal. Ciao... Today I watched surgery on a pt with Arnold-Chiari I malformation (the posterior cranial fossa is too small to accomodate the cerebellum so it begins to herniate out of the foramen magnum) and associated severe syringomyelia. We may think it's rare, but at the institute, they see it quite frequently and perform a few of these surgeries per week. Also watching the surgery with me were an italian second year resident (he barely ever scrubs and never operates) and a Chinese Neurosurgeon who is at the institute getting his masters in Neurophysiology (EEG, etc... except he thinks this is really boring so he always coming into the OR to watch instead... how does that work out??!). HI EVERYONE!!! :-) It's currently a dreary, rainy, and chilly day here in Milan... so rather than venturing around the city like I usually do after work, I decided to come home, get cozy with a glass of wine, and finally start my travel blog! As all or most of you know, I'm here on a scholarship given out by my medical school (the Philadelphia College of Osteopathic Medicine) studying Neuroradiology and Neurosurgery at the Instituto de Neurologico C. Besta. The institute is super busy and English-speaking neuroradiologists are tough to come by. As a result, the first two weeks I worked from about 10am-2pm and then went exploring around Milan. A wonderful week end in Varese with my cute friend Myriam. We had a nice walk in the woods on the mountain and had the opportunity to see from far away Laggo Maggiore. We had the joy to sit on a nice coffee terrace on the mountain and watch the sun set sitting in the sun. Just great!... read more

We finally took off - the big day (forgive me throughout, I had forgotten that Italian keyboards have keys placed in odd places). The flight from Atlanta to London was fine, except that we did not sleep nearly enough. I forgot had difficult it can be to sleep sitting up - in five minute incraments before ones head falls to the side, waking one up. In London we checked in over three hours early. We then proceeded to wait in a security check line for the next three hours. As our departure time came dangerously close (and no one in the line had any idea how far it wrapped around and where it ended) we kept trying to find some airport folks to help get us through on time. back in time to when I first arrived... Dave and I *did* have a little scare that I'd miss my plane, but thankfully I didn't and the trip went smoothly. No one HUGE or smelly or hairy or annoying sitting next to me... no wailing babies... I actually had two seats to myself and curled up in a ball and slept the whole time. After plane, train, and cab I arrived at my current address, Viale Piave 11 where I thought I was subletting an apt. Turns out this 60-some year old was just renting a room/bathroom. Here's the kicker, though... he's a LIFE COACH! Apparently he used to be this big time business man when became interested in meditation and quit his job all together. This week I had a couple of new experiences. For example I went to see Babel at a theater new the Duomo. Mauro and a couple of his friends invited me to go see it. The movie was in Italian but it was a good experience because I had seen the movie in English before I left so I could try and understand Italian from comparing it to the first time I saw it. The movie theater was interesting because unlike in the States, you have to buy a specific seat in the theater before you enter, and there is a computer diagram of the remaining seats for the line to see before you pay. This week I was able to walk around town more and learn my way around the streets better. I have been able to search around the Duomo area in more depth as well as being able to get around the area called Romolo (which is where Mark lives and where a lot of students live) and Brerra, which is a strange area inhabited by gypsies (they come out at night and tell fortunes on the streets, along with people who gamble at tables that get set up on the streets). Brerra was also the place we went to watch the super bowl. To see the super bowl, we went to a club named fiori oscuri, which is a posh nightclub frequented by models most nights, and is owned by a guy from New Mexico. It was ... read more

So far, my time has been taken up by going to Alberto's office on weekdays, coming home to eat at 19.00, then having my 30th cup of coffee for the day, which is basically a true estimate. After that I have just been doing a couple of things around the house and going to bed, I dont think I am totally on Milanese time yet. I did go out with Mark (Fuller) the second night I was here, we went with Mark's friends to a party for students from the top fashion school in Milan, everyone was dressed way to nicely. We went with two of his American friends, a Puerto Rican (which I guess is American too) and his Italian roommate. Milan.. Why Milan you ask? Well.... It’s all about Fashion..Lots of wine and pizza.. Italians....should be a great place to visit... hmm... high expectations set for Milan... We managed to get fairly inexpensive train tickets from Zurich and off we were on a 7am train ride The ride from Zurich to Milan was arguably the most beautiful train ride..The Swiss Alps were breathtaking and the small villages covered with Snow were postcard worthy. Talar and I did take turns dosing off for about an hour or so but David was a diligent photographer through the entire 6 hours. We got to Milan around 1p and were pretty impressed with the Milan train station.. It reminded me of a grander Grand Central.
