I LOVE BRISBANE! Though I understand my other blog was a bit of a downer, it should in no way reflect the amazing experience I had in Brisbane. I had the perfect 3 day weekend in a sunny paradise. I arrived late on Thursday morning to the Kookaburra Inn, a lovely bed and breakfast style inn just north of the main city center. My room was clean and comfortable and there was a full kitchen and sunny porches to relax on. Not to mention - WIFI!!! Oh how I've missed my WIFI, much more than television. I haven't watched TV in a month and couldn't miss it less. But the luxury of the internet... that's one which has been hard. Note: The following contains a bit of soul searching on a post vacation come-down. Not the same tone of my other blogs. Enter the caverns of my mind at your own risk. I sit in the blue and grey checkered TigerAir seat 16C in flight from Brisbane to Adelaide... Depressed. Depressed. Depressed. I am coming off of an incredible 3 day weekend in sunny Brisbane, I have the row to myself, have plenty of snacks and entertainment material, and a large black woman in a wonderfully loud yellow dress and matching turban is singing herself to sleep across the aisle. Prime fodder for a relaxing, enjoyable, and musical flight home. And yet I feel I may burst into tears. So what gives? I should know by now. I must say, I'm being treated very well here in Oz! Friday night, Trevor, a product rep and entrepreneur that I met at a Corin dinner, picked me up in his (ahem) Ferrari. We spent some time at his lovely home with his partner of 9 years, LeeAnn. ("Partner" in Oz just indicates long term girlfriend. Really no one says boyfriend or girlfriend here. Actually, I don't think anyone really casually dates here! Long term relationships at a young age, definitely status quo). We shared some nice wine and some olives and cheeses I had picked up at the Central Market. (I have a feeling there are going to be a lot of sidebars. I think I have figured out how Australians dine. Weekend was fun. Friday I went to the repatriation hospital (it was a former veterans hospital) and worked with Mr. Chris Brown (in Australia, Mr indicates surgeon and Dr indicates medical doctor. So technically, I'm Mr. Palanca) who is a foot and ankle surgeon. It was nice to see a few foot cases, since I've been only doing trauma this far. We worked until 5:30 or so (Which seemed quite late in Aussie standards from the way the nurses were talking). So then I met up with Ai and we went to the central market for dinner. The market is amazing! There is produce, and coffee shops, and artisan shops with nuts and cheeses and mushrooms - all actually reasonably priced. On Fridays there is also live music. I thought it may be helpful to give my very simplistic view of orthopaedic health care in Australia from my limited experience thus far at Flinders Medical Centre. Orthopedics in Australia Intern=intern Registrar=resident Fellow=fellow Consultant=attending Australian undergraduate and medical school is a lot like the US. 4 years undergraduate, 4 years medical school. They are given placement for their intern year which is a guaranteed spot, and this is at one hospital. After that year, they become an RMO. And they can either be on a medicine track, or a surgical track. This is when things get tricky. After a couple year, then they apply for orthopedics, which is extremely competitive. If they do not get a spot, they then have to do another year and apply again (this is when they are a service registrar). ... read more

It's been pretty fantastic so far! The arrival was a bit rocky. Mechanical issues on the plane led to a delay in LA for 24 hours (though I got to have dinner with Chris and Kyla) and then when I arrived in Sydney a day late, I found they had left my luggage behind in LA. Everyone at the airport found this quite a nonissue and laughed about it. They said no problem, they would just ship it to me. However... I had no address yet. So they said no problem, they would just call me. But I had no phone number. So I was less than thrilled, but I got contact information and went to catch my connection to Adelaide. Settled in at work. I spend most of my time in theatre, though when theatre is slow I've been in clinic. The case load here is quite similar to Stanford - young people getting injured on bikes and car accidents, and old people falling and breaking their wrists and hips. What's amazing is how quickly things get booked. A hip fracture will come into the ED in the morning and then we will operate on it during the afternoon. I've seen some really interesting stuff in clinic too. There was a young boy who fell on a flower pot and then started growing MOLD in his knee. Mold! There are only a few instances of this ever published. So his knee was irrigated with pool cleaner and now he's on some heavy duty antifungal medication. I ... read more

Today was the first full functioning day at the hospital, given it's a Monday. We were packed. All clinics were packed and we ran two ORs all day. Ortho was relatively light in the OR. We had one case - a revision of an external fixator, wash out, and flap coverage of the open tibia fracture that had come in my first night. Dr. Thomas, a Haitian orthopaedic surgeon who trained with French surgeons in trauma for 6 years, also scrubbed. We got excellent alignment of the fracture and he taught me how to do a full thickness skin flap for coverage. It was amazing. I wanted to put a rod in the tibia, but he said the infection rates here in Haiti are too high. The patient will need a skin graft in a week ... read more

Haiti Missed first flight. Rerouted thru FLa. Sat next to great fam w really smart kid with anxiety problems. Dad said when a sports team loses he would worry about if a player was going to get fired and if they had family. Got on flight to port au prince- amazing shit show. People brought on TVs and trophies that wouldn't fit under their seats but they closed the cabin door. Had to finagle things in, took 30 min. Hilarious. Sat next to wonderful Haitian woman who left Haiti 43 yrs ago and moved to Montreal to become a cosmetologist. Now lives in Miami. Her husband wants to move back to Haiti. Said America is the land of opportunity but she was now too old and too tired to keep up. Wants to make room for ... read more
