I have 4 free weeks in December and I'm planning to travel Middle-East. I would like to go from Istanbul to Cairo in 4 weeks. Is it realistic? If not, on which part of this itinerary should I focus on?

Reply to this Hello Etienne and welcome to the forum!
4 weeks will be a rush to see all of this - I have been to all the countries (except Israel) that you mention and you are bypassing a lot of places - and in Egypt and Turkey you are only seeing the capitals - there is so much more to see in each country. For example, I had 7 weeks in Turkey, almost 2 weeks in Syria, the same time in Jordan and 4 weeks in Egypt - and in hindsight, one could easily spend longer in Syria and I didn't even get to the Sinai Peninusla in Egypt!
I am not sure how you will get between Istanbul and Aleppo - I hope it is not by road, as it will take you more than one day to get there. Once you get to the trio of countries (Syria, Israel, Jordan) it is easy to move around and your itinerary looks a fair one for that part, but there are other highlights in Syria - for example Palmyra and Krak des Chevaliers.
Also be aware that winter is not the best time to travel through parts of Turkey (snow) or Syria (rain). If you wish to follow the best weather and to save yourself for spreading your travels too thinly - I'd recommend Israel, Jordan and Egypt for this trip.
Reply to this it is the good idea that startıng your trıp from ıstanbul and i saw you your holiday program and i think it is very good but i dont thınk 4 day enough to see all amazıng place in ıstanbul. if you have question about istanbul i can help you just ask enjoy your trip
Reply to this Given the time contraints, your itinerary isn't bad, although it's a shame you aren't allowing yourself more time for Turkey. The other poster is right; it will take you more than a day to get from Istanbul to Aleppo. I would suggest cutting your time in Amman down to two nights at the most and I don't think Aleppo is worth stopping for anything more than a bus transfer. The extra time would be much better spent elsewhere. You might want to consider more than 1 day in Petra, and you surely cannot miss a visit to Crac de Chevalier in Syria. It might be possible to do this in route from Aleppo to Damascus, but you might have to spend a night in Homs or Hama. It's all subjective, but if I had 4 weeks, I'd spend at least 10 days in Turkey, 6 in Syria, 6 in Jordan, and maybe 3 or 4 in Cairo. I haven't been to Israel so I have no suggestions for there.
