Today the weather was sunny and hot although I had to spend most of my time inside. I turned down the opportunity to see a spinal surgery to go back on the labor and delivery unit. Sadly it was a very slow morning on the unit. I was only able to observe one delivery right before leaving for lunch. A lot of the moms on the unit had complications/high risk pregnancies. Some of these complications/situations included epilepsy, twins, SVD, and hydrocephaly. The hydrocephalus case was very interesting. The doctors plan for the birth was for it to be vaginal! Once the baby descends into the birth canal, the doctor will tap into the babies head and release the fluid so that the baby can safely make its way down the canal and into the world. Another day at Nakornping Hospital, I was thrilled to be able to visit the OR as I find it interesting and fun to experience the anatomical/ surgical potential. When offered the opportunity to go today, I was very quick and assertive in volunteering as I was eager to see a Thai operating room. Seven of us went and we had to change out of our Messiah uniforms to the hospitals’ gowns and . . . sandals! In the operating room I wore normal scrubs, a hair cover, a face mask, and sandals, which were an odd combination for me. When I think sterility and protecting ourselves from pathogens and spills, I would think most people would avoid open toe shoes at all costs. January 8th Back to work (kind of). This morning, we participated in the nursing students' morning ceremony. It is focused around respecting the teachers, the flag, and Buddha. Christianity is growing in Thailand so there is a moment of silence for Christians who would like to pray which I thought was interesting and thoughtful. We got a full tour of the college and saw the different simulation rooms. They are very similar to the ones at Messiah. After the college tour, we toured the hospital that we will be working in called Nakornping Hospital. We saw about 7 different units, including men's medical, women's medical and surgical, trauma, emergency, pediatrics, and labor and delivery. Today my alarm went off at 6 am for I had to get ready for an all-day clinical. Breakfast was served at 7. Today was fried chicken, chicken broth with carrots, rice, stir-fry morning glory (one of my favorites) and fruit. We attended morning ceremony at 8 am and then headed off to Nakornping Hospital. Students were escorted to their assigned units where they met up with their preceptors. This morning I chose to observe the Male Medical Ward. It was definitely a busy place. They had 45 patients and 9 of those patients were on ventilators. The acuity level was pretty high. The nursing model for this unit was a 2-team approach. Group A & Group B each consisted of 1 general nurse, 1 medication nurse, 1 treatment nurse and 1 helper or aide. Most ... read more

First day on the L&D unit, and what a day! I saw three deliveries total, which was really awesome. Overall I had a good experience. Thai nursing students are all trained to be midwives, so the nursing students were the ones delivering the babies. I thought this was so neat. I was actually kind of jealous because I have to go back to school for at least 3 years to be a midwife in the States. The staff were all very friendly, including the nursing instructors from BCNC and Chiang Mai university. They were both willing to answer our questions and also asked us about ourselves. There was probably a total of 15 or 20 nursing students on the labor unit, which made it really hard for me to help out and get hands on experience. ... read more

Today was our first “full day” in the NPK hospital on our units. Melissa and I went to the Women’s Medical unit upon arriving and were greeted by our preceptor. She was very kind, gave us water bottles right away, and brought us to the conference room to talk with her. She shared how she is proud of her unit and stated that though her staff knew little English, we would have a good experience. It became evident she was leaving for a meeting for the morning and I was nervous we would be unable to communicate without her and would be lost in translation, literally. Today I woke up at 0645 and felt tired but I was so happy I was able to sleep through the night! I put on my clinical uniform for the first time in a while and was ready for the day at the hospital. We went down and ate breakfast which was some rice with chicken and some delicious glazed donuts! It was cool to have some breakfast food that was similar to what we would have at home. After eating we met our Thai buddies and walked down to have the morning ceremony with the students. January 7th More meat for breakfast, but this time it was fried chicken. (It was finger-licking' good.) After breakfast, we went to Doi Suthep, which is a temple on top of a mountain overlooking the city of Chiangmai. We stopped at an overlook on the way up the mountain, but there were clouds covering everything. As soon as we got to the top, the clouds cleared and we hurried out of the bus. There were people and dogs everywhere. I even saw some cats lounging on cars. We headed up to the temple only to find that ahead of us were 300 steps. Onwards and upwards. Today is our travel day to Chiang Mai, Thailand. I’m going to be honest, I’ve been panicking a little bit. It’s been an emotional overload processing going abroad once again. I haven’t left the country since the fall of 2015 during which I studied abroad in Rwanda, Africa. My experience was rough to say the least, but if I can manage to make it a few days without food (and no money to obtain food) as well as deliver stillborn babies day after day, then I can do just about anything. I am worried about compassion fatigue as well as an overload of unmanageable emotions. It has taken me a fairly long time to adjust and feel safe in my surroundings. Today was beautiful. Blue skies, sun, low humidity and a slight breeze. Everyone slept well and except for an upset stomach and a cold, everyone is feeling good. This morning for breakfast we had Chinese Buddy Donuts, Salad, Mango, Bananas and Chicken Rice Soup. Once again, not disappointed with the meal selection. We attended the morning ceremony where each student introduced themselves in Thai and in English. Some of the students are very comfortable with their introductions; however, there are still a few students struggling with the phrase. The Thai introduction is “De Chan Chew (name), kah”. This simply means “my name is ….”. Each student has a Thai nickname and this year the names are flowers. After the morning ceremony, we had our group picture taken.
