Jim and I have cringed a number of times watching women with babies go upstairs on buses, in front of vehicles etc and seen babies almost decapitated but somehow it seems to work.

They would probably cringe at the way we strap our babies up all the time in buggies, car seats, walking reins, high chairs etc.
